Timothy Francis Jones Foundation Shares Why They Gave to Adler Community Health Services

Adler Community Health Services (ACHS) received a generous grant of $5,000 from the Timothy Francis Jones Foundation, an organization focused on advocating for proactive healthcare, especially among men of color. The grant is in support of ACHS’s partnership with Complex High Admission Management Program at Northwestern Memorial Hospital, which helps patients with a high reoccurrence of hospital admissions.
