Primary Care Services Program

Adler Community Health Services (ACHS) partners with primary care practices across Chicagoland to provide an array of team-based clinical services including psychotherapy, trauma-informed care, substance abuse counseling, wellness training, health behavior modification, and rehabilitational support.

Our Committment to Excellence in Practice

ACHS' multidisciplinary team of practitioners, doctoral interns and administrators are trained with the highest ethical and clinical practice standards. Our training program which facilitates the integration of behavioral health primary care in underserved communities is recognized and partially supported by the Health Resources & Services Administration (HRSA), and has received a renewal to continue advancing comprehension of comorbidities, risk factors and treatments, and improving patient outcomes in collaboration with medical and behavioral health providers.

Near North Health Service Corporation (NNHSC)
Operating nine comprehensive health centers and two supplemental nutrition sites across Chicagoland, Near North Health Service Corporation (NNHSC) takes a team-based approach in providing comprehensive clinical services for individuals in underserved communities. NNHSC offers a range of primary care services, including individual, group and family psychotherapy; crisis intervention; nutrition education; social support programs; and assessment for mental health and behavioral issues, substance abuse disorders and learning disabilities.

 Aunt Martha's Health & Wellness
Aunt Martha’s is a family of more than 900 staff and volunteers. We are health care providers, social workers, educators, counselors, and advocates. We are as diverse as the people and communities we serve. We are parents who know what it is to have a sick, hurting or scared child. We have relied on the help of others to overcome our own challenges. We honor people’s strengths, resourcefulness and resilience. We offer compassion. We provide supportive, professional care. We are a team – skilled and specialized. From the psychiatrist treating children in downstate Illinois, to the dental hygienist caring for a homeless person in Kane County; and from the therapist helping a youth in crisis on the streets of Chicago, to the doctor delivering a baby in Kankakee. Our staff are the best and the brightest in the field. We are all Care Coordinators – intensely dedicated to our patients and clients, connecting each person with exactly what they need, from the services our organization provides to the specialized care offered by our collaborating partners.

Northwestern Hospital 
The Complex High Admission Management Program (CHAMP) at Northwestern Hospital is an innovative model of care consisting of a small team of providers who establish continuity relationships with high-utilizer patients. CHAMP strives to provide effective, high value care via a longitudinal relationship-based care model through provider continuity, intensive case management, and personalized care plans.
